Looks like a New Yorker cover from the time, of a bunch of Kids trick or treating a scary house in the neighborhood.

Yeah making noise. That type has a metal plate for the two balls./beads to bounce off, the ones Clod linked have the wood drilled out and a slot cut to make a clacking sound.
